Copyright Permission Request Form
To request Copyright permission from TransplantInformers please make sure you have first read our Copyright Information and Permissions policy. Then submit your request following these two steps:
1)Please fill out and submit the form below.
2)You must also send a sample or mock-up for the item for which you are seeking a logo or copyright approval. Please also advise the date by which you need approval. Requestors must submit a sample prior to approval. Send your sample to transplantinformers@gmail.com.
Requests cannot be processed unless the above directions are followed in full.
